I'm leaving tomorrow night to get my work Visa, and I forgot that I needed an HIV test. Anyone know where I can get one with same day results, or next day results?

Try Invitro. Their site says they can do it in 2 hours: http://www.invitro.ru/analizes/for-doctors/545/2267/?sphrase_id=4119310

It is two years ago since I last took a medical and three since I had to submit a HIV test for a visa. To my understanding, the requirement is to have medical insurance.

I don't know of anyone among my collegues who have to submit medicals for a visa.

HIV tests are required for all work and study visas. I've never seen any exceptions for that.

Medicals are required whenever you extend a work/study visa, but it's possible to avoid taking the tests for a study visa if you don't live in a dormitory.

I'm not sure about highly paid specialists - they may be exempted from the requirements.

I was upgraded to the specialist visa in early 2011 and did not have to take a medical for that. The last medical I had was in 2010. Some of my team have standard one-year permits, but they too have not taken a medical since 2010. There are currently in the course of renewing their work visas (end of July) and again this year have not had to submit a medical. I have friends in other companies and they tell me the same.

I am not sure what the case is. Do some companies just get around all of this bureaucracy with a bribe or what?

For instance, what is the case with registration? I am registered at my office, which is fine for a Specialist visa. But my work colleagues on the standard work visa are also registered at the office. Also, when I go abroad I no longer bother with registration -- my company said I don't need to hand it in if I travel again within 3 months.

If you've stayed with the same company and on the same kind of visa, the HIV test isn't required. It's always required at foreign consulates when you get the initial 90 day work visa.
I checked again, and it seems that full medical tests aren't required for work visas extensions any more, only for the initial conversion from a 90-day single entry visa into a 1-year ME one. I think HQS visa holders are exempt from all kinds of medical tests.

HQS visa holders don't have to regster for the first 90 days, so that is legal. It's also legal for employees to be registered at the company address instead of their residential address. Study and business visas work the same way. You always have 2 choices for registration - either where you are actually staying, or at the address of the inviting organisation.

I came on HQS visa earlier this year and I did need HIV test, but no other medical.

In London, got one for free and asked them for a letter the same day, which was very kind as I was on my way to the visa place.

or just type into google or yahoo -free HIV tests and your city- and you will get the places that do an express test and for free.
http://www.yellowpages.com/vienna-wv/hiv-testing) I took Vienna my home town and got that, so it should be in your place as well.
If you are still in Moscow, the Sklivosovsky Hospital does it also in 2 hours.
